This dataset is built from the CMS National Plan and Provider Enumeration System (NPPES) monthly file — the federal registry every billing provider must maintain — with the NPPES deactivation report applied so retired NPIs are excluded, specialties resolved against the NUCC provider taxonomy, and Medicare participation joined from the CMS Care Compare Doctors and Clinicians national file. Nothing here comes from a provider's own website; every record is the provider's own registered entry.

No. CMS does not publish an email or website in these files. 99% of facilities have a phone.

Which facilities are left out?

Facilities whose certification has ended are left out: 32,137 of them. Every row is an active CCN.

How does it differ from the hospital and nursing home lists?

Those lists each hold one type with a short set of columns. This one holds all 15 types in one file, with every field CMS publishes for each.

Does it include facilities outside the U.S.?

270 hospitals outside the U.S. are certified and kept: 263 in Canada and 7 in Mexico. Their country column says CA or MX, and they are never placed in a U.S. county or ZIP code.
