112,228 US dental practice locations with street address, phone and the practice's declared specialty. There is no national register of dental PRACTICES — state boards license dentists, not premises — so this has to be assembled, which is exactly why it is worth buying rather than reproducing. The file covers general dentistry, orthodontics, oral and maxillofacial surgery, endodontics, periodontics, pediatric dentistry, prosthodontics and dental public health, each tagged so a buyer selling to one specialty is not paying for the rest. One number here is deliberately smaller than it could be: the source enumerates billing entities, and a single office routinely carries several of them — one per owning PLLC, one per specialty line, one left over from an acquisition. The worst address in the raw data carried 22. Those are collapsed to one row per physical location, so 112,228 means 112,228 places you can visit, not 132,801 tax identities. Where several registrations share an address the row says so. Dental is consolidating as fast as veterinary medicine, so distributors, practice-management software vendors, equipment and imaging manufacturers, staffing agencies and the private-equity buyers rolling up practices all segment on exactly this. Why is there no official list of dental practices?

Because state dental boards license individual dentists, not the premises they work in. A handful of states register facilities separately; most do not. So there is no register to download and the practice-level universe has to be built — the same reason a veterinary clinic list has to be assembled.

Is this practices or individual dentists?

Practices. It is built from organization registrations only, which return entities like "Turners Family Dentistry, Ltd", never individual practitioners. We checked that rather than assuming it, because an NPI-derived list returning individual dentists is the obvious way this goes wrong.

Does it say which practices are DSO-owned?

Partly, and we would rather be straight about the limit. About 7% declare a parent organization in the source, and those are named. The rest are blank — which means not declared, NOT independent. A full ownership column means scraping each DSO's own locator, which is on the roadmap. Do not filter for independents on a blank in this column yet.

How complete is it?

A practice that never registered for a provider identifier is absent, and a group that registered one identifier across several offices appears once. It is the most complete assembled view we can refresh weekly, but no one can honestly quote a completeness percentage against a universe nobody publishes.

Why not just filter the free CMS NPPES file myself?

You can — it is public, free of license restrictions, and it is where this file comes from. Budget for a roughly 1 GB compressed download and for four transformations that are easy to get wrong: filtering to organization registrations (NPI-2) rather than individual dentists; using the full taxonomy set, including 261QD0000X for dental clinics and 204E00000X for physician-side oral surgery, which a 1223-branch filter misses entirely; reading the practice location rather than the mailing address, which is often a PO box or a billing service; and collapsing the several registrations a single office carries down to one row. That last step alone takes 132,801 registrations to 112,228 real locations. How does this compare to Census County Business Patterns or a commercial database like Data Axle?

County Business Patterns (NAICS 621210, Offices of Dentists) is the authoritative count of dental establishments and publishes no names at all, so it is the right way to audit a list's completeness by county and cannot be used to build one. Commercial compilers such as Data Axle do sell one row per office with firmographics NPPES has never held — employee and revenue estimates — but on annual or per-record terms that usually restrict redistribution, and their dental coverage is itself substantially NPPES-derived, so it is not an independent second opinion on the same universe.
