All 14,690 Medicare and Medicaid certified skilled nursing facilities in the United States, with street address, phone, bed count, ownership structure and the CMS star rating. Ownership is the field that decides who you are actually selling to: 5,221 are for-profit limited liability companies, 4,679 for-profit corporations, 2,311 non-profit corporations, and the remainder individually or partnership owned. That distinction drives purchasing authority, capital access and consolidation risk, and it is published nowhere in a scraped directory. Medical supply and pharmacy service providers, staffing agencies, food service, therapy contractors, EHR and care-management vendors, and the private equity and REIT investors active in senior care all work from this file. Refreshed weekly from CMS.
