US Federally Qualified Health Centers
U.S. Location Dataset // 18,944

18,944 federally qualified health center sites across the United States, from the federal health center program's own data, with street address, phone and site details. FQHCs and look-alikes deliver primary care to underserved communities and are funded and tracked federally, which is why a complete site-level file exists at all. Note the number: this is service delivery SITES rather than the roughly 1,400 grantee organizations, and sites are what matters operationally because each is a physical clinic that needs supplies, staff, equipment and connectivity. Health centers are significant buyers of primary care equipment, dental and behavioural health services, pharmacy and 340B services, and practice-management software, and they buy at both the site and organization level. CSVFormat

All 18,944 records, as published — the complete set rather than a sample. That matters most for coverage and gap questions: put together the same list from partial searches and every location you miss reads as a gap that is not there, so the answer is wrong rather than merely late. Assembling the U.S.-wide coverage by hand also means one query per area against sources that cap what they return per search.

?Frequently Asked Questions

Why 18,944 when people say there are about 1,400 health centers?

Because 1,400 is the number of grantee organizations and this file is their service delivery sites. A single organization commonly runs a dozen or more clinics, plus school-based and mobile sites. Sites are the right unit for anything physical.

What is the 340B connection?

Health centers are covered entities under the 340B drug pricing program, which makes them distinctive pharmacy and pharmaceutical customers. This file gives you the sites; 340B registration detail is a separate federal dataset we can join for you.

Does it include look-alikes?

Yes, where the federal data identifies them. Look-alikes meet the same requirements without receiving the grant funding and operate the same way commercially.
