26,222 state-licensed assisted living and residential care communities, assembled state by state because no federal agency licenses them. This is the hard half of senior care and the reason this dataset exists: CMS covers skilled nursing, so a nursing home list is a federal file download, but assisted living, memory care and residential care homes are licensed by fifty separate state agencies with fifty separate formats. Every record carries the facility name, street address, phone and the state license, with licensed capacity where the state publishes it. Senior living is consolidating fast and is one of the most active private equity and REIT categories in healthcare, so operators, medical and mobility suppliers, food service, staffing, pharmacy services, insurance and software vendors all need the operating universe rather than the subset that advertises. Refreshed weekly.
All 26,222 records, as published — the complete set rather than a sample. That matters most for coverage and gap questions: put together the same list from partial searches and every location you miss reads as a gap that is not there, so the answer is wrong rather than merely late. Assembling the U.S.-wide coverage by hand also means one query per area against sources that cap what they return per search.

Why is this harder to get than a nursing home list?
Because there is no federal register. Skilled nursing facilities are Medicare-certified, so CMS publishes them; assisted living is purely state-licensed, which means fifty agencies, fifty formats, and several that publish nothing in bulk at all. Assembling it is the work, and that is what you are buying.
Does it cover every state?
It covers the states that publish their licensing register in a usable form, which is most but not all of them. We would rather name the coverage than claim fifty. Ask about a specific state and we will tell you exactly what is in the file.
Is memory care separated out?
Where a state licenses it as a distinct category, yes, in the facility type column. Many states license memory care as an endorsement on an assisted living license rather than a separate class, in which case it appears as assisted living.
