# US Dental Assistant Providers — U.S. location dataset

All 2,846 licensed providers in the U.S., as one CSV: 64 fields per record, 51 states, file built September 14, 2026. $49 one-time, with up to 10 real rows free before you buy.

Canonical URL: https://locationlists.com/data/us-dental-assistant

2,846 licensed healthcare providers at their practice address. 100% carry a street address, 100% a telephone number, 47% a state license number, 100% a specialty from the NUCC taxonomy. Built from the CMS National Plan and Provider Enumeration System (NPPES) monthly file, the NPPES deactivation report, the NUCC provider taxonomy and the CMS Care Compare Doctors and Clinicians national file. Every deactivated NPI is removed — 351,911 of them — rather than shipped as an empty row, and the address is the practice location, never the mailing address. These rows are individual providers, so the record is a licensed person at the address where they practise. Buy the organization file instead if you need businesses only. Coverage is 51 states and territories.

## The figures

- Records: 2,846 licensed providers (2,808 US)
- States: 51
- Coverage: U.S.
- Fields: 64
- Price: $49 one-time, CSV download, commercial license (https://locationlists.com/license)
- File built on September 14, 2026. Not on an automatic refresh schedule.
- File last modified: 2026-09-14

The file has all 2,846 licensed providers across 51 states in one download. Building the same list by searching one area at a time tends to miss places, and a missed place looks like a gap in coverage that is not really there.

## Questions

### Where does this data come from?

The CMS National Plan and Provider Enumeration System — the federal NPI registry — plus the NUCC provider taxonomy and the CMS Care Compare clinician file. All three are published by the government and free to download; what you are buying is the parsing, the taxonomy decode, the deactivation cleanup and the cut. Every vendor selling an NPI-based list is working from the same files.

### Are these businesses or individual people?

Individual providers — a licensed person at the address where they practise. CMS publishes NPPES for public use with the practice address attached. If you need businesses only, buy the organization file or one of the practice cuts instead.

### Is this the same data Definitive Healthcare or IQVIA sell?

The registry underneath is the same public CMS file. What those vendors add on top is their own layer — hospital affiliations they research, executive contacts, claims-derived volumes, purchasing intent. If you need that, buy them. If you need the registry itself as a file you own, this is it, at a fraction of a subscription.

### Does this include email addresses?

No, and no honest NPI-based product does — NPPES has no email field. What it does have is the Direct secure-messaging address that some providers register for clinical data exchange, and those ship in their own column, labelled, because they are not marketing addresses. Any vendor selling you a 'physician email list' built on NPI data appended those emails from somewhere else.

### How current is it?

File built on September 14, 2026. It is not on an automatic refresh schedule, so check the build date above before you buy.

### What format is the download?

A single UTF-8 CSV with a header row, one row per NPI, ready for Excel, a CRM import or a database load. Commercial licence, one-off purchase, no subscription.

### How is this data cleaned?

Every rebuild runs the same steps and counts what each one changed. For the current file, rebuilt September 14, 2026 from CMS NPPES: Merged 0 duplicate listings that had the same business name, street address and ZIP code, keeping every filled-in detail from both (September 14, 2026). Unified 45 city names written more than one way within the same ZIP code (for example with and without a space or an abbreviation) to the US Census Bureau spelling (September 14, 2026). Corrected 3 misspelled city names to the US Census Bureau place name for their ZIP code (September 14, 2026). Added map coordinates to 35 listings that had none, from the US Census Bureau address geocoder or, where the street address could not be matched, the centre of the ZIP code (September 14, 2026). Set the county and its federal county code on 2,767 listings, matched to the US Census Bureau county list from the county the source records, the listing's map point, or its ZIP code area (September 14, 2026). Checked every map point against the state in its address and re-placed 36 points that fell outside that state (September 14, 2026). Flagged 127 addresses that are incomplete, a PO box, or missing a house number, in an address quality column (September 14, 2026). Checked that the row count, state count and field completeness published for this file were measured from the file itself (September 14, 2026).

### Isn't this free on CMS NPPES?

The underlying records are public on CMS NPPES. What you pay for is the work between that public release and this file: unified 45 city spellings, corrected 3 misspelled city names, added coordinates to 35 listings, added the county to 2,767 listings, re-placed 36 misplaced map points, flagged 127 incomplete addresses and measured 2,846 rows. The current file was rebuilt September 14, 2026 from CMS NPPES. Doing it yourself means downloading the full release, repeating each of those steps, and repeating them again every time the source updates.

### How complete is each field?

Measured on every row of the current file, the share of rows that carry each key field: phone 100%, street address 100%, city 100%, state 100%, ZIP code 100%, latitude 99%, longitude 99%. 47 of 66 columns are filled on at least half the rows.
