45,843 US outpatient rehabilitation locations with street address, phone, discipline mix and subspecialty. Nearly half — 22,598 — carry a facility-level classification, meaning the source itself calls them a clinic rather than us inferring it from a practitioner's credential, and that distinction is marked on every row so a buyer can take only the confirmed sites if they want. Physical therapy, occupational therapy and athletic training are together on purpose, because they are the same clinics: the operators consolidating this sector run PT, OT and hand therapy out of one site under one brand, and hand therapy in particular is classified under physical therapy or occupational therapy purely according to which credential the treating therapist holds. Splitting them would cut real clinics in half. Each row shows its discipline mix, so a PT-only list is one filter away. Subspecialties are named where the practice declared them — orthopedic, sports, neurology, pediatrics, geriatrics, hand, ergonomics, electrophysiology, neurorehabilitation and more. Therapy assistants are excluded, because that is a credential a person holds rather than a place, and speech-language pathology is deliberately out of scope.

## The figures

- Records: 45,843 locations
- States: 52
- Coverage: U.S.
- Fields: 21
- Price: $89 one-time, CSV download, commercial license (https://locationlists.com/license)
- File built on September 15, 2026. Not on an automatic refresh schedule.
- File last modified: 2026-09-15

The file has all 45,843 locations across 52 states in one download. Building the same list by searching one area at a time tends to miss places, and a missed place looks like a gap in coverage that is not really there.

## What we did to this data

Unified 841 city spellings, corrected 48 misspelled city names, added coordinates to 45,807 listings, added the county to 45,807 listings, flagged 259 incomplete addresses and measured 45,843 rows. Rebuilt September 15, 2026 from npiregistry.cms.hhs.gov.

## How complete the key fields are

phone 100%, street address 100%, city 100%, state 100%, ZIP code 100%, latitude 100%, longitude 100%. 19 of 21 columns are filled on at least half the rows.

## Ask this list a question with a link

No connector and no account; each link answers with the matching count, preview rows and the price for just that part.

## Questions

### Is this physical therapy only, or occupational therapy too?

Both, plus athletic training, and every row states its discipline mix so you can filter to PT-only in one step. They are together because the clinics are the same clinics — ATI, Athletico, Ivy Rehab and Upstream run PT and OT from one site — and because hand therapy is split across the two classifications by credential rather than by clinic.

### How many rows have a named subspecialty?

About 15%. Most practices declare only the base classification and never a subspecialty, so we report what is there rather than inventing granularity. Where it is declared you get real detail: orthopedic, sports, neurology, pediatrics, geriatrics, hand and more, including combinations.

### Does it identify the corporate operators?

Only where the clinic declared a parent organization, which is about 7% of rows. A blank means not declared, not independent. Naming the platforms properly requires scraping each one's locator and is on the roadmap.

### How is this data cleaned?

Every rebuild runs the same steps and counts what each one changed. For the current file, rebuilt September 15, 2026 from npiregistry.cms.hhs.gov: Merged 0 duplicate listings that had the same business name, street address and ZIP code, keeping every filled-in detail from both (September 15, 2026). Unified 841 city names written more than one way within the same ZIP code (for example with and without a space or an abbreviation) to the US Census Bureau spelling (September 15, 2026). Corrected 48 misspelled city names to the US Census Bureau place name for their ZIP code (September 15, 2026). Added map coordinates to 45,807 listings that had none, from the US Census Bureau address geocoder or, where the street address could not be matched, the centre of the ZIP code (September 15, 2026). Set the county and its federal county code on 45,807 listings, matched to the US Census Bureau county list from the county the source records, the listing's map point, or its ZIP code area (September 15, 2026). Checked every map point against the state in its address and re-placed 0 points that fell outside that state (September 15, 2026). Flagged 259 addresses that are incomplete, a PO box, or missing a house number, in an address quality column (September 15, 2026). Checked that the row count, state count and field completeness published for this file were measured from the file itself (September 15, 2026).

### Isn't this free on npiregistry.cms.hhs.gov?

The underlying records are public on npiregistry.cms.hhs.gov. What you pay for is the work between that public release and this file: unified 841 city spellings, corrected 48 misspelled city names, added coordinates to 45,807 listings, added the county to 45,807 listings, flagged 259 incomplete addresses and measured 45,843 rows. The current file was rebuilt September 15, 2026 from npiregistry.cms.hhs.gov. Doing it yourself means downloading the full release, repeating each of those steps, and repeating them again every time the source updates.

### How complete is each field?

Measured on every row of the current file, the share of rows that carry each key field: phone 100%, street address 100%, city 100%, state 100%, ZIP code 100%, latitude 100%, longitude 100%. 19 of 21 columns are filled on at least half the rows.
