41,513 US eye care locations with street address, phone and declared specialty, covering both optometry and ophthalmology. Both halves are here on purpose: an OD and an MD are different professions, but they are one commercial market — the consolidators buy both, and anyone selling frames, lenses, diagnostic equipment, surgical devices or practice software calls on both. Every row is tagged as optometry, ophthalmology, or a combined OD+MD practice, so a buyer who only wants one can filter rather than be denied the rows. Specialties are broken out where declared: pediatrics, cornea and contact lens, low vision rehabilitation, vision therapy, occupational and sports vision, and retina. Eye care has no facility-level provider category at all — we checked, and no such classification exists — so this is built from the practitioner registrations held by organizations, then collapsed to one row per physical address. That collapse matters: the raw source held 48,303 registrations at 41,513 real locations, and selling the larger number would have overstated the footprint by 16%.

## The figures

- Records: 41,513 locations
- States: 52
- Coverage: U.S.
- Fields: 21
- Price: $89 one-time, CSV download, commercial license (https://locationlists.com/license)
- File built on September 15, 2026. Not on an automatic refresh schedule.
- File last modified: 2026-09-15

The file has all 41,513 locations across 52 states in one download. Building the same list by searching one area at a time tends to miss places, and a missed place looks like a gap in coverage that is not really there.

## What we did to this data

Unified 778 city spellings, corrected 41 misspelled city names, added coordinates to 41,487 listings, added the county to 41,487 listings, flagged 257 incomplete addresses and measured 41,513 rows. Rebuilt September 15, 2026 from npiregistry.cms.hhs.gov.

## How complete the key fields are

phone 100%, street address 100%, city 100%, state 100%, ZIP code 100%, latitude 100%, longitude 100%. 19 of 21 columns are filled on at least half the rows.

## Ask this list a question with a link

No connector and no account; each link answers with the matching count, preview rows and the price for just that part.

## Questions

### Why are optometrists and ophthalmologists in the same file?

Because the buyers treat them as one market and the consolidators acquire both. They are tagged separately on every row, so filtering to one is trivial — whereas if we split the file, anyone who needed both would have to buy twice and dedupe the overlap themselves.

### Does it identify EyeCare Partners, MyEyeDr and the other groups?

Only where the practice itself declared a parent organization, which is about 6% of rows. A blank means not declared, not independent. A real ownership column requires scraping each platform's own locator and is on the roadmap.

### Are retail optical chains included?

Where they operate as a registered eye care practice, yes. A pure retail optical counter with no practice attached is not an eye care practice and is not in scope.

### How is this data cleaned?

Every rebuild runs the same steps and counts what each one changed. For the current file, rebuilt September 15, 2026 from npiregistry.cms.hhs.gov: Merged 0 duplicate listings that had the same business name, street address and ZIP code, keeping every filled-in detail from both (September 15, 2026). Unified 778 city names written more than one way within the same ZIP code (for example with and without a space or an abbreviation) to the US Census Bureau spelling (September 15, 2026). Corrected 41 misspelled city names to the US Census Bureau place name for their ZIP code (September 15, 2026). Added map coordinates to 41,487 listings that had none, from the US Census Bureau address geocoder or, where the street address could not be matched, the centre of the ZIP code (September 15, 2026). Set the county and its federal county code on 41,487 listings, matched to the US Census Bureau county list from the county the source records, the listing's map point, or its ZIP code area (September 15, 2026). Checked every map point against the state in its address and re-placed 0 points that fell outside that state (September 15, 2026). Flagged 257 addresses that are incomplete, a PO box, or missing a house number, in an address quality column (September 15, 2026). Checked that the row count, state count and field completeness published for this file were measured from the file itself (September 15, 2026).

### Isn't this free on npiregistry.cms.hhs.gov?

The underlying records are public on npiregistry.cms.hhs.gov. What you pay for is the work between that public release and this file: unified 778 city spellings, corrected 41 misspelled city names, added coordinates to 41,487 listings, added the county to 41,487 listings, flagged 257 incomplete addresses and measured 41,513 rows. The current file was rebuilt September 15, 2026 from npiregistry.cms.hhs.gov. Doing it yourself means downloading the full release, repeating each of those steps, and repeating them again every time the source updates.

### How complete is each field?

Measured on every row of the current file, the share of rows that carry each key field: phone 100%, street address 100%, city 100%, state 100%, ZIP code 100%, latitude 100%, longitude 100%. 19 of 21 columns are filled on at least half the rows.
