81,562 active Medicare and Medicaid-certified facilities from CMS's Provider of Services file for the third quarter of 2026. 14,696 are nursing homes and 13,049 federally qualified health centers. 12,589 are home health agencies, 7,522 dialysis facilities and 6,800 hospitals. 6,665 are ambulatory surgical centers, 6,327 hospices, 5,816 rural health clinics and 5,196 ICF/IID homes. The rest are outpatient therapy providers, portable X-ray suppliers, residential psychiatric facilities, rehab facilities, mental health centers and organ procurement organizations. Each row has the CMS Certification Number, facility type, phone (99%), certification dates, ownership, beds and staff counts. It keeps every field CMS publishes for each type. Nursing homes add their Care Compare star ratings, staffing hours and penalties. 99.6% have a map point. CMS publishes no email or website.

### Where does this data come from?

From CMS's Provider of Services files for the third quarter of 2026, both the hospital file and the iQIES file for every other type, read on 28 October 2026. Nursing homes add CMS's Care Compare provider file.

### Is there an email or website for each facility?

No. CMS does not publish an email or website in these files. 99% of facilities have a phone.

### Which facilities are left out?

Facilities whose certification has ended are left out: 32,137 of them. Every row is an active CCN.

### How does it differ from the hospital and nursing home lists?

Those lists each hold one type with a short set of columns. This one holds all 15 types in one file, with every field CMS publishes for each.

### Does it include facilities outside the U.S.?

270 hospitals outside the U.S. are certified and kept: 263 in Canada and 7 in Mexico. Their country column says CA or MX, and they are never placed in a U.S. county or ZIP code.
