Canonical URL: https://locationlists.com/data/fqhc-list

19,030 federally qualified health center sites across the United States, from the federal health center program's own data, with street address, phone and site details. FQHCs and look-alikes deliver primary care to underserved communities and are funded and tracked federally, which is why a complete site-level file exists at all. Note the number: this is service delivery SITES rather than the roughly 1,400 grantee organizations, and sites are what matters operationally because each is a physical clinic that needs supplies, staff, equipment and connectivity. Health centers are significant buyers of primary care equipment, dental and behavioural health services, pharmacy and 340B services, and practice-management software, and they buy at both the site and organization level. Refreshed weekly.

## The figures

- Records: 19,030 locations
- States: 52
- Coverage: U.S.
- Fields: 21
- Price: $69 one-time, CSV download, commercial license (https://locationlists.com/license)
- File built on October 18, 2026. Rebuilt weekly on an automatic schedule.
- File last modified: 2026-10-18

The file has all 19,030 locations across 52 states in one download. Building the same list by searching one area at a time tends to miss places, and a missed place looks like a gap in coverage that is not really there.

## Questions

### Why 18,944 when people say there are about 1,400 health centers?

Because 1,400 is the number of grantee organizations and this file is their service delivery sites. A single organization commonly runs a dozen or more clinics, plus school-based and mobile sites. Sites are the right unit for anything physical.

### What is the 340B connection?

Health centers are covered entities under the 340B drug pricing program, which makes them distinctive pharmacy and pharmaceutical customers. This file gives you the sites; 340B registration detail is a separate federal dataset we can join for you.

### Does it include look-alikes?

Yes, where the federal data identifies them. Look-alikes meet the same requirements without receiving the grant funding and operate the same way commercially.

### How is this data cleaned?

Every rebuild runs the same steps and counts what each one changed. For the current file, rebuilt October 18, 2026 from data.hrsa.gov: Merged 102 duplicate listings that had the same business name, street address and ZIP code, keeping every filled-in detail from both (October 18, 2026). Unified 310 city names written more than one way within the same ZIP code (for example with and without a space or an abbreviation) to the US Census Bureau spelling (October 18, 2026). Corrected 0 misspelled city names to the US Census Bureau place name for their ZIP code (October 18, 2026). Shortened 17,790 ZIP codes to the five-digit code every address filter expects, keeping the delivery-route segment in a column of its own (October 18, 2026). Added map coordinates to 18,902 listings that had none, from the US Census Bureau address geocoder or, where the street address could not be matched, the centre of the ZIP code (October 18, 2026). Set the county and its federal county code on 16,808 listings, matched to the US Census Bureau county list from the county the source records, the listing's map point, or its ZIP code area (October 18, 2026). Checked every map point against the state in its address and re-placed 0 points that fell outside that state (October 18, 2026). Flagged 353 addresses that are incomplete, a PO box, or missing a house number, in an address quality column (October 18, 2026). Checked that the row count, state count and field completeness published for this file were measured from the file itself (October 18, 2026).

### Isn't this free on data.hrsa.gov?

The underlying records are public on data.hrsa.gov. What you pay for is the work between that public release and this file: merged 102 duplicate listings, unified 310 city spellings, shortened 17,790 ZIP codes, added coordinates to 18,902 listings, added the county to 16,808 listings, flagged 353 incomplete addresses and measured 19,030 rows, and a fresh rebuild every week. The current file was rebuilt October 18, 2026 from data.hrsa.gov. Doing it yourself means downloading the full release, repeating each of those steps, and repeating them again every time the source updates.

### How complete is each field?

Measured on every row of the current file, the share of rows that carry each key field: phone 100%, street address 100%, city 100%, state 100%, ZIP code 100%, latitude 99%, longitude 99%. 18 of 21 columns are filled on at least half the rows.
