Canonical URL: https://locationlists.com/data/dialysis-center-list

All 7,490 Medicare-certified dialysis facilities in the United States, with CMS Certification Number, street address, phone and ownership. Dialysis is the most concentrated corner of American healthcare, with two operators controlling the large majority of clinics, which makes the independent and hospital-based remainder a specific and reachable target for anyone selling equipment, supplies, water treatment, transport, staffing or software. Every patient on dialysis attends three times a week, so these are high-frequency, high-consumable sites with predictable purchasing. Certification means the file is authoritative and joins to the CMS quality data published for the same facilities. Refreshed weekly.

## The figures

- Records: 7,490 locations
- States: 52
- Coverage: U.S.
- Fields: 22
- Price: $39 one-time, CSV download, commercial license (https://locationlists.com/license)
- File built on October 18, 2026. Rebuilt weekly on an automatic schedule.
- File last modified: 2026-10-18

The file has all 7,490 locations across 52 states in one download. Building the same list by searching one area at a time tends to miss places, and a missed place looks like a gap in coverage that is not really there.

## What we did to this data

Unified 122 city spellings, added coordinates to 7,477 listings, added the county to 6,717 listings, flagged 68 incomplete addresses and measured 7,490 rows. Rebuilt October 18, 2026 from data.cms.gov.

## How complete the key fields are

phone 100%, street address 100%, city 100%, state 100%, ZIP code 100%, latitude 100%, longitude 100%. 21 of 22 columns are filled on at least half the rows.

## Ask this list a question with a link

No connector and no account; each link answers with the matching count, preview rows and the price for just that part.

## Questions

### Does it identify the big chains?

The ownership information CMS publishes is included, which in most cases identifies whether a facility belongs to a large operator, a hospital system or an independent. If you need a clean chain rollup we can provide it as an enrichment.

### Are home dialysis programs included?

Facilities certified to train and support home dialysis appear as certified facilities. Patients dialysing at home are not locations and are not in any location dataset.

### Why is this a good prospecting list?

Because consumption is relentless and predictable. Every center runs the same treatments on the same schedule every week, so supply, water treatment, maintenance and staffing needs are continuous rather than project-based.

### How is this data cleaned?

Every rebuild runs the same steps and counts what each one changed. For the current file, rebuilt October 18, 2026 from data.cms.gov: Merged 0 duplicate listings that had the same business name, street address and ZIP code, keeping every filled-in detail from both (October 18, 2026). Unified 122 city names written more than one way within the same ZIP code (for example with and without a space or an abbreviation) to the US Census Bureau spelling (October 18, 2026). Corrected 0 misspelled city names to the US Census Bureau place name for their ZIP code (October 18, 2026). Added map coordinates to 7,477 listings that had none, from the US Census Bureau address geocoder or, where the street address could not be matched, the centre of the ZIP code (October 18, 2026). Set the county and its federal county code on 6,717 listings, matched to the US Census Bureau county list from the county the source records, the listing's map point, or its ZIP code area (October 18, 2026). Checked every map point against the state in its address and re-placed 0 points that fell outside that state (October 18, 2026). Flagged 68 addresses that are incomplete, a PO box, or missing a house number, in an address quality column (October 18, 2026). Checked that the row count, state count and field completeness published for this file were measured from the file itself (October 18, 2026).

### Isn't this free on data.cms.gov?

The underlying records are public on data.cms.gov. What you pay for is the work between that public release and this file: unified 122 city spellings, added coordinates to 7,477 listings, added the county to 6,717 listings, flagged 68 incomplete addresses and measured 7,490 rows, and a fresh rebuild every week. The current file was rebuilt October 18, 2026 from data.cms.gov. Doing it yourself means downloading the full release, repeating each of those steps, and repeating them again every time the source updates.

### How complete is each field?

Measured on every row of the current file, the share of rows that carry each key field: phone 100%, street address 100%, city 100%, state 100%, ZIP code 100%, latitude 100%, longitude 100%. 21 of 22 columns are filled on at least half the rows.
