112,228 US dental practice locations with street address, phone and the practice's declared specialty. There is no national register of dental PRACTICES — state boards license dentists, not premises — so this has to be assembled, which is exactly why it is worth buying rather than reproducing. The file covers general dentistry, orthodontics, oral and maxillofacial surgery, endodontics, periodontics, pediatric dentistry, prosthodontics and dental public health, each tagged so a buyer selling to one specialty is not paying for the rest. One number here is deliberately smaller than it could be: the source enumerates billing entities, and a single office routinely carries several of them — one per owning PLLC, one per specialty line, one left over from an acquisition. The worst address in the raw data carried 22. Those are collapsed to one row per physical location, so 112,228 means 112,228 places you can visit, not 132,801 tax identities. Where several registrations share an address the row says so. Dental is consolidating as fast as veterinary medicine, so distributors, practice-management software vendors, equipment and imaging manufacturers, staffing agencies and the private-equity buyers rolling up practices all segment on exactly this.

## The figures

- Records: 112,228 locations
- States: 52
- Coverage: U.S.
- Fields: 21
- Price: $149 one-time, CSV download, commercial license (https://locationlists.com/license)
- File built on September 14, 2026. Not on an automatic refresh schedule.
- File last modified: 2026-09-14

The file has all 112,228 locations across 52 states in one download. Building the same list by searching one area at a time tends to miss places, and a missed place looks like a gap in coverage that is not really there.

## What we did to this data

Unified 1,839 city spellings, corrected 191 misspelled city names, added coordinates to 112,137 listings, added the county to 112,137 listings, flagged 494 incomplete addresses and measured 112,228 rows. Rebuilt September 14, 2026 from npiregistry.cms.hhs.gov.

## How complete the key fields are

phone 100%, street address 100%, city 100%, state 100%, ZIP code 100%, latitude 100%, longitude 100%. 19 of 21 columns are filled on at least half the rows.

## Ask this list a question with a link

No connector and no account; each link answers with the matching count, preview rows and the price for just that part.

## Questions

### Why is there no official list of dental practices?

Because state dental boards license individual dentists, not the premises they work in. A handful of states register facilities separately; most do not. So there is no register to download and the practice-level universe has to be built — the same reason a veterinary clinic list has to be assembled.

### Is this practices or individual dentists?

Practices. It is built from organization registrations only, which return entities like "Turners Family Dentistry, Ltd", never individual practitioners. We checked that rather than assuming it, because an NPI-derived list returning individual dentists is the obvious way this goes wrong.

### Does it say which practices are DSO-owned?

Partly, and we would rather be straight about the limit. About 7% declare a parent organization in the source, and those are named. The rest are blank — which means not declared, NOT independent. A full ownership column means scraping each DSO's own locator, which is on the roadmap. Do not filter for independents on a blank in this column yet.

### How complete is it?

A practice that never registered for a provider identifier is absent, and a group that registered one identifier across several offices appears once.

### Why not just filter the free CMS NPPES file myself?

You can — it is public, free of license restrictions, and it is where this file comes from. Budget for a roughly 1 GB compressed download and for four transformations that are easy to get wrong: filtering to organization registrations (NPI-2) rather than individual dentists; using the full taxonomy set, including 261QD0000X for dental clinics and 204E00000X for physician-side oral surgery, which a 1223-branch filter misses entirely; reading the practice location rather than the mailing address, which is often a PO box or a billing service; and collapsing the several registrations a single office carries down to one row. That last step alone takes 132,801 registrations to 112,228 real locations. Our guide at /guides/list-of-dental-offices-in-the-us specifies the whole build, including the taxonomy codes, whether you buy the file or not.

### How does this compare to Census County Business Patterns or a commercial database like Data Axle?

County Business Patterns (NAICS 621210, Offices of Dentists) is the authoritative count of dental establishments and publishes no names at all, so it is the right way to audit a list's completeness by county and cannot be used to build one. Commercial compilers such as Data Axle do sell one row per office with firmographics NPPES has never held — employee and revenue estimates — but on annual or per-record terms that usually restrict redistribution, and their dental coverage is itself substantially NPPES-derived, so it is not an independent second opinion on the same universe.

### How is this data cleaned?

Every rebuild runs the same steps and counts what each one changed. For the current file, rebuilt September 14, 2026 from npiregistry.cms.hhs.gov: Merged 0 duplicate listings that had the same business name, street address and ZIP code, keeping every filled-in detail from both (September 14, 2026). Unified 1,839 city names written more than one way within the same ZIP code (for example with and without a space or an abbreviation) to the US Census Bureau spelling (September 14, 2026). Corrected 191 misspelled city names to the US Census Bureau place name for their ZIP code (September 14, 2026). Added map coordinates to 112,137 listings that had none, from the US Census Bureau address geocoder or, where the street address could not be matched, the centre of the ZIP code (September 14, 2026). Set the county and its federal county code on 112,137 listings, matched to the US Census Bureau county list from the county the source records, the listing's map point, or its ZIP code area (September 14, 2026). Checked every map point against the state in its address and re-placed 0 points that fell outside that state (September 14, 2026). Flagged 494 addresses that are incomplete, a PO box, or missing a house number, in an address quality column (September 14, 2026). Checked that the row count, state count and field completeness published for this file were measured from the file itself (September 14, 2026).

### Isn't this free on npiregistry.cms.hhs.gov?

The underlying records are public on npiregistry.cms.hhs.gov. What you pay for is the work between that public release and this file: unified 1,839 city spellings, corrected 191 misspelled city names, added coordinates to 112,137 listings, added the county to 112,137 listings, flagged 494 incomplete addresses and measured 112,228 rows. The current file was rebuilt September 14, 2026 from npiregistry.cms.hhs.gov. Doing it yourself means downloading the full release, repeating each of those steps, and repeating them again every time the source updates.

### How complete is each field?

Measured on every row of the current file, the share of rows that carry each key field: phone 100%, street address 100%, city 100%, state 100%, ZIP code 100%, latitude 100%, longitude 100%. 19 of 21 columns are filled on at least half the rows.
